This is the second time Dr. Jacquelyn Sammons has saved us from an expensive ER trip with her amazing suture skills and selfless willingness to open up her office to us on her day off.BACKSTORY: Later in the morning our toddler pulled on an American flag decoration 🇺🇸 that was anchored under a metal clock and, literally, ‘clocked’ himself🤦‍♀️😢 leaving a gash so deep that even the urgent care doctor we initially took him to wasn’t comfortable gluing or suturing and told us to go to the ER.We checked our local ER wait times and they were already at 2-4 hours.Assuming doctor was enjoying the holiday out of town, but desperate to avoid the ER, we texted Dr. Jacquelyn’s concierge line as a last ditch effort as we got some lunch at home and started packing up all 5 kids for a long ER visit.We heard back in 15 minutes, met her at the office 30 minutes after that, and in less than an hour we were heading back home.But what’s even better than the time saved was that the toddler wasn’t traumatized by the big, strange, exhausting hospital experience.🐠He got to see the fish tank he loved (he sees it when we visit for school physicals, monthly osteo adjustments or IV treatments). And he recognized doctor so everything felt ‘normal’ and safe.She used numbing cream and waited before using the initial injected local anesthesia. She let him watch a show to distract him instead of insisting on sterile area hospital protocols that would have covered his face with cloth (urgent care doctor even mentioned this procedure saying that the hospital would have more people/ways to ‘hold him down’ while suturing with the cloth covering his face).Crying and trauma was at a minimum. Comfort and peacefulness was top priority. She had her 16 year old daughter shadowing her and learning medical terminology, doubling the moment as a teaching opportunity (which I love!). So incredibly blessed by the level of care that comes from having a one-on-one relationship with a doctor we trust and share in the same values, willing to spend her holiday helping us in our hour of need.We have been concierge patients with her for over 4 years and only become more and more grateful for her heart for healing and service. My first visit to Dr Cortez’s office was a pleasant and satisfying experience. The staff is friendly and welcoming. Dr Cortez is kind and gentle, yet very professional. She patiently answered all my questions. The treatment was exactly what I needed, and I felt much improved the very next day. I highly recommend a visit to her office!

You won’t be stopped from getting the care you need because your Dr is dictated by ins co. limitations. You won’t be rushed by the Dr. or limited to “whatever your ins co will authorize”. Be prepared to be treated by someone who will listen and care while providing the best services offered in medicine today. After visiting 5 Drs within my ins network and being told “I just need to live with the symptoms of menopause”, I was about to give up and accept depression, weight gain, mood swings and low energy. But I found Dr. Sammons at Transformation Health and Wellness and my health and quality of life has never been better. I had a vitamin IV and what a great experience. Lovely, comfortable lounge chairs. A solid consult with Dr Simmons who is fantastic. Easiest IV I have had inserted and Ive had a lot. Customized to my needs, the doctor offered exactly what I was in need of. I got to lay back in my lounge chair with hot tea and a warm blanket during the drip. A day later, no headache after 2 weeks of one, more energy after needing to nap every morning (No nap today!) and more clarity than Ive had in weeks. Ill be back.

Wow...what an AMAZING Dr., office and staff! I am a caregiver and had contact with a family member that came down with Covid-19. I was referred to this office by family and friends and I couldnt have received faster, better care! I called the office, they got me right in and within 15 minutes had my NEGATIVE results! While there I noticed ALL their other services that they provide and I was amazed! I am a disabled person myself and battle daily chronic pain. I am against/afraid of the surgery and narcotics that have been recommended and prescribed and have been looking for a healthier alternative. Needless to say, I have found a place for treatment and cant wait to begin! I would highly recommend Transformation Health and Wellness Center...but suggest you check them out for yourself! Absolutely the best doctor around! She treats each patient individually and doesn’t just follow the text book. She digs until she finds the answers. I really don’t like visits to the see the doctor, I usually get nervous and anxious. I have Kaiser as my primary care provider and it’s fine but it takes a long time for an appointment to see my primary Dr. So I wind up seeing a different Dr. or a P.A. every time and have to describe my symptoms or issues multiple times.Welp, problem solved! Thanks to Dr. Cortez, my visits are more personalized and precise. The office setting is neat and more intimate then Kaiser. The staff is very welcoming and professional. Dr. Cortez has provided top notch care for my wife, three kids and myself.Do not hesitate if your undecided.

Doctor Sam went out of her way to give me her time. It was Friday and this clinic closes at 3pm. I called around 4:45pm and left a voice mail. I had symptoms of COVID-19 so I wanted to get tested ASAP. Doctor called me back 5:45pm and gave me appointment for 6pm same day on Friday. When I got there she really took her time to listen to me, check with all proper measures and got me tested. Excellent experience.

This is my 3rd time seeing Dr. Cortez for different reasons. Each visit was more than satisfactory. She explained what was going on in the way that was very easy for me to understand.Dr. Cortez is an OBGyn turned Doctor of Osteopathy who provides a lot of services that you would not necessarily find in a medical office. Services that seem minor but are really important and provides immediate relief without having to wait at an Urgent Care or ER.In need of Vitamin C or D3 boost? Maybe a quick shot for some type of pain? Or maybe a near severing of a finger (like me)?Whatever it is, she makes it pain free and simple.Her staff is attentive and provides great customer service. The office is easy to find, just off the 91/55/57!

I had Covid with 102-103 fever I was running for 14 days then got double pneumonia was doing horrible. Yesterday, I went to the office to get a flu shot and was informed that they now offer a chemical-free facial using cold therapy. I decided to try it and am so pleased that I did. If we had more doctors like Dr. Cortez-Sammons healthcare wouldn’t be a joke. She takes the time to hear what is going on. She then does all she can to find a solution. Prior to meet Dr. Cortez I avoided going to the doctor because of how I was treated. Then a friend who knew I needed a dr and knew my fears told me I had to give her a chance. This center was so kind and genuine even with this being my first visit and they walked me through the check in process so easily. Very good customer service and I would highly recommend them to anyone. Very fast service. They took me back into a room the second I walked into the building and took care of me. This place is awesome I had my antibody checked here and all the nurses were very friendly and patient seeing as I’m a huge wuss when it comes to getting a shot or especially blood taken. I used to go to another office close by and compare to this office I can say it was going from zero to 1000 time better.
